New Feature Drops: Intake Form Updates
Intake forms can now request image or PDF uploads, plus images can be included as visuals on the form and forms can be submitted by people who do not have a customer account yet π
We know that intake forms are a vital part of any appointment heavy business, especially in the healthcare and wellness sectors. The ability to receive important information about a clientβs health and needs prior to your first session allows your business to operate smoothly and efficiently, and empowers your staff to be prepared. This is why we have been committed to developing intake forms on Momence, ensuring this feature works optimally for what you need.
With this newest release drop, you can now do the following:
Require or request an image upload along with any part of the form. For example, if you need customers to send you their health insurance card, or a photo of their license for insurance purposes. Additionally, it might make sense to have a client send you a photo of the injury or issue that they are having.
You will be able to detail the label about what you are requesting to be uploaded, and you can also choose to include a sample image within the prompt.
Require or request a PDF upload along with any part of the form. Similar to the above, you may want to request a PDF of required items if that is the format you need to submit to an insurance agency, or if it is best for saving within your records.
Intake form fields can have images uploaded as part of the label. You can now include your own image within any field or question on your intake forms. For example, if youβd like to include a sample image of a health insurance card, highlighting the numbers that you need to see on it. Or, maybe you provide an image of the human body with labeled points so that customers can detail exactly where they are feeling pain, like in the example below
Intake forms can be submitted by people who do not yet have a customer account. We recognize that, oftentimes, intake forms are the first step someone takes prior to booking an initial appointment. To make this process as seamless as possible, if someone without a customer account goes to fill out an intake form, they will be prompted to complete their profile and create an account once they press submit on the form (versus needing to navigate to a profile creation page first).
PS - not seeing intake forms on your account? Navigate to Apps & Integrations > App store > activate Intake forms then you will find them under Customers > Intake forms!
